First Time Patient Information

Please arrive 15 minutes before your first appointment. If you have not filled out the new patient forms that will be emailed to you after scheduling your first visit, allot additional time. Wear comfortable, loose fitting, stretchy clothing. Bring your insurance card, a picture ID and any pertinent imaging (X- Rays, MRI's, CT scans), that you may already have.

In-Network insurance plans include, but may not be limited to, BCBS PPO, Aetna, United Healthcare, Humana, Cigna, several Amita HMO plans, Medicare and the VA Community Care program.

Humana Gold Plus HMO REQUIRES A REFERRAL FROM YOUR PRIMARY CARE PHYSICIAN
